Change Account Information |
| Complete the Address – Name Change Form (PDF) if you need to change your address, phone number or name. |

Stop Payment Form |
| Complete the Stop Payment Form (PDF) if you need to place a stop payment on a check or ACH debit entry. If you wish to place a stop payment on a VISA debit card transaction, please contact our Member Services department for further information at 800-851-5132 or by email. |

| Complete the Written Statement of Unauthorized Debt if you are disputing an unauthorized or improper ACH debit to your account. |

Direct Deposit/Payroll Deduction |
| Sign up for Direct Deposit or Payroll Deduction when you work for one of our sponsor companies. Don’t work for a sponsor company or don’t see the form listed, call your payroll department and they can help you. You’ll need to provide them with your Credit Union account number and our routing number. It’s 311080573. |
